RELEASE AND WAIVER OF LIABILITY HARBOUR DANCE CENTRE

THE FOLLOWING DESCRIBES THE TERMS ON WHICH HARBOUR DANCE CENTRE OFFERS YOU ACCESS TO OUR SERVICES. 
I wish to participate in a dance and/or fitness class at Harbour Dance Centre. As a condition of Harbour Dance Centre agreeing to allow me to participate in such a dance and/or fitness class, I hereby confirm and agree to the following:
1. I recognize that the dance and fitness classes of the type I intend to participate in at Harbour Dance Centre require physical exertion which may be strenuous and may cause physical injury, and I am fully aware of the risks and hazards involved.
2. I understand that it is my responsibility to consult a physician prior to and regarding my participation in any classes I attend at Harbour Dance Centre. I represent that I am physically fit and have no medical condition that would prevent my full participation in any such fitness or dance class.
3. I agree to take full responsibility for any risks, injuries or damages known or unknown which I incur, or may incur, as a result of participation in any class at Harbour Dance Centre.
4. I knowingly and voluntarily waive any and all claims I have, or may at any time in the future have, against Harbour Dance Centre or any of the instructors, agents, or employees, for any injury or damage that I sustain as a result of my participation in a class at the Harbour Dance Centre.

Required:

I have read the above release and waiver of liability and fully understand its contents. I voluntarily agree to the terms and conditions stated above. I understand that this is NOT REFUNDABLE OR TRANSFERABLE unless acompanied by a Doctor's note.

Harbour Dance Centre Progressive Workshops: Are a commitment to a course.

There is a pre-registration deadline price before a certain date.

After the pre-registration date $10 may be charged up to 1 course.

After a workshop has started, there is no pro-rating of the fees.

If a student registers in more than 2 courses, 10% off each course is applied.

Workshop refund policy: If before the workshop starts, a student is unable to attend, HDC will either refund the amount minus a $25 processing fee or transfer the whole amount to the student's account at HDC.
After the first class there are no refunds unless there is a medical reason accompanied by a doctor's note. A refund would be subject to the $25 processing fee and classes attended.

Make-up class: In some circumstances, 1 make-up class may be given per workshop session. Students must write "make-up" when signing in to the drop-in class. 1 make-up request must be made within 1 week from their absence from the class. Email: info@harbourdance.com
